It’s passive. But it works quite well. You will need acoustic guitar amp or PA to get a good sound, records an acceptable sound into DAW. Better than on board piezo in my electro acoustics but nothing beats a mic for recording.

Q: Can you remove it ok after attaching it without damaging the instrument ? I have a very old guitar and would not want to damage the varnish.

It did not leave a trace on 2 of my satin finish ukes, but it slightly damaged the surface of my glossy soprano uke. However, you must be careful. The sticky tape that is provided is very strong. When you want to remove the pickup, you need to twist it rather strongly, and all that force exerted on it can actually damage the device. After I had removed it from 2 instruments, the whole thing split open in the middle of a gig. It still worked luckily. I fixed it with sticky tape, but I am not happy about it. If I had known, I would have used a weaker sort of tape, such as masking tape or blue tak, as recommended by some users.

Q: Can this be connected to an earphone as monitor?

It's passive pickup, need acoustic guitar amp or PA to get a good sound, records an acceptable sound into DAW, if you use PC recording, you need a external sound card and preamplifier DI

The jack socket on the pickup is 2.5mm. It came with the jack to jack lead.
