Q: Will this help a severe diaper rash with open sores?

I found it to be a very good diaper rash cream - though I don't think I ever had to put it on open sores

Q: Can this be used for adults?

Yes. I used it on my eczema and the weepy red skin immediately dried up and now I use Fluonicadae, butwithout Metanium, it would have been difficult.
