Could I use this transformer plugged into a zwave adapter to trigger a standard a 30A alarm siren?

Probably not, alarm sirens are usually 12 volts DC, this transformer is AC. You would probably need a relay and a DC power supply

Is there a way to know which screw is positive and which one is negative

The output is AC (alternating current). There is neither positive nor negative to such a circuit as there is in a DC output.
