No but I bought a flip up cover for mine. It does come with a carry pouch.

What is the eye relief (for someone who wears glasses)?

Eye relief for the 8x36 is 18mm, which is probably the best choice for someone wearing glasses (and the reason I chose that particular model)

Does this monocular have an "infinity"? i.e. is there a distance beyond which you no longer have to focus? If so, how far out is it?

I focused mine at 100 yards and anything further is in focus. I have viewed items at a closer distance, say to 25 yards, and it is still in focus. Hope this helps.
