what is the difference in 3 or 4 rows as far as cleaning teeth and would you need to floss after using either one anyway?

The 3 row toothbrush is for cleaning the gum line using the Bass method. The 4 row toothbrush is for polishing the teeth. And yes flossing is still needed.

how often should brush be replaced?

I think it depends on how often you brush your teeth, really. I got a new one after a few months. It is soft.
