What are the natural flavors made from?

You would have to contact the company. No one who has bought the product on Amazon will know that and they don't have to provide that info on the label.

Does it need to be refrigerated after opening? Once opened, what is its shelf life: in the fridge and in room temperature, respectively?

It says refrigerate after opening. It also says, in French, Best Before May. The year did not print! I would guess the sugar content would mean it keeps indefinitely in the fridge, like maple syrup would. When it gets old, it would probably show its age by developing hard crystals around the opening of the bottle. Hope that makes sense.

Could you confirm if this is a glass bottle still? I placed the order, but cancelled because the “purchase image” showed plastic bottle.
