Q: does it measure wind speed and how does it do it?

With this anemometer, the red cup is used to count the number of rotations, or turns, which is then used to calculate wind speed.

Q: what is the distance from the inner edge of one rotating cup to the one on the other side?

The distance between opposite cups, measured from the inside edge, is approximately 120mm (4.75")

Q: How heavy is this - will it blow over on my deck railing?

It's only 3-4 oz so yes it will likely be blown over in a strong gust. Best to secure it by placing a weight on the base.
